For patients with health insurance plans: be advised if you have a deductible that you have not met - you will need to book an appointment under General Visit (paying out of pocket). The fee will be added to your deductible once the claim is processed.

 

If you only have a co-pay and your insurance plan has no deductible OR you have met your deductible - please book under  General Visit with Health Insurance Plan. We do charge $35.00 co-pay at the time of booking. The co-pay will be adjusted or refunded if appropriate once the claim is processed. Always ask questions if any.
